This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free. COFMC dental offers the following services: fillings, extractions, crowns, bridges, full dentures, partial dentures, simple cleanings (adult and children), deep cleanings for adults (scaling and root planing), fluoride treatment, root canals, invisalign, and comprehensive orthodontics. Hours: Monday, Tuesday, Wednesday, Thursday 8 AM - 5 PM Call for appointment; walk ins accepted at 7:45 AM and 12:45 PM. If more than 10 minutes late for appointment, patient may be rescheduled for another day. Fees: Accepts Medicare, Sooner Care, Medicaid, insurance; sliding scale Must bring insurance card (private insurance, Medicare, Soonercare, Medicaid), information on all medications being taken, proof of income if using sliding scale.
