This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free. Offers a broad range of primary care services provided by a dedicated team of physicians, dentists, family nurse practitioners, physician assistants, social workers, nurses, and support staff.  Sliding-fee schedule for those who have high deductible health plans or no insurance based on the household  income and family size. Hours: Monday - Friday: 8:00 am - 5:00 pm.
