Step 1:  Call 344-5156 to schedule an eligibility appointment on: Monday & Wednesday between 1:30 pm – 3:00 pm OR Tuesday & Thursday between 4:00 pm – 6:00 pm  Step 2:  You will need: -Photo I.D. -Proof of Residence (utility bill, lease, etc.) -If employed, one recent pay stub -If unemployed, last 30 days of paystubs  Step 3:  If you are given a dental appointment, DO NOT MISS IT or you will not be eligible to come back for six (6) months.   Do you need a filling? Fillings are scheduled by Appointments ONLY First, you will need to schedule an eligibility appointment by calling 344-5156.  Dates for eligibility appointments are on: Monday & Wednesday between 1:30 pm – 3:00 pm OR Tuesday & Thursday between 4:00 pm – 6:00 pm  Next, you will be contacted by the dental coordinator, Danielle, when there is an opening for fillings.
