The sliding fee discount is confidential and persons applying for the discount must provide documentation of income, such as employee paycheck stubs, government benefits, etc. Depending on qualifications, patients may be eligible for a 100%, 75%, 50%, or 25% discount on services. Hours: Monday-Friday 8:00 am to 5:00 pm.
