This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free. This location provides dental services. Payments accepted: Private Insurance, Medicaid, CHIP, Medicare (where applicable), Medicaid-on-hold for patients 2 years and under, Cash, and Uninsured assistance available based on family size and income (a sliding fee). CHSA provides care on a sliding fee basis. The sliding fee schedule is based upon family size and income. Please speak with staff to see if you are eligible for this service.
