This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free. Dental Services:  Cleanings  Sealants  Check-ups and X-rays  Fillings  Extractions  Oral Cancer Screening  Healthy Gum Care  Root Canal Treatment  Limited Oral Surgery Patients who do not have health insurance may be seen as self-pay patients through the CHCBH Discount Program. Self-pay patients will be charged for services based on their income and family size. Patients should bring proof of income (for the last 30 days) to their first appointment. Hours:  Monday - Thursday 8:00 am - 6:00 pm.
