This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free. Offers dental health services to children through age seventeen. SMCHC offers a sliding fee program for patients who qualify based on annual gross household income and size. Applications are required at a minimum of annually. Qualifying income levels are adjusted in March of every year based on most current poverty table levels. Work Hour   Monday through Friday 8:00 a.m. to 5:00 p.m.
