This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. To qualify for the sliding discount, you must fill out a form (these can be picked up at any of the offices) and bring proof of income into any of our offices. A minimum payment of $30 per Medical visit and 50% per Dental visit, is expected at time of services. While they are not a free clinic, they are able to provide reduced costs to those who qualify while providing quality health care to all.
