Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free. The Choptank Community Health Sliding Fee program covers payment for necessary medical & dental care and medications purchased through the Choptank Health Prescription Centers. Services:  May include: a dental screening, cleaning, fluoride treatment (which may possibly be applied two times during the school year), sealants and if needed, referrals for prescriptions and dental emergencies.
