This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free. Hours: Mon - Fri 8am--5:15 pm, Wed until 7:30pmAffordable Dental Care to those in the community who need it.Application Instructions:For fee discounts, provide proof of income less than 200% of Federal Poverty Level.Fees:Dependent upon insurance co-pays and/or sliding fee discounts if income is under 200% of Federal Poverty Level.Dental Services Include:-Dental Exams and radiographs-Preventive Services (cleanings)-Restorative (fillings) – Composit (white) and amalgam (silver), night guards and bleaching-Crown and Bridge-Removable Prosthetics (Partials and Dentures)-Minor Oral Surgery-Selected Root Canal Therapy
