Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free. Cornell Scott-Hill Health Center is a federally qualified community health center established in 1968 in a collaboration between the community and Yale School of Medicine. Sliding Fee Scale The Cornell Scott-Hill Health Center seeks to make its services more affordable for uninsured, low-income patients. If you are uninsured and a low-income patient, you may qualify for reduced fees based on your income. This is called a sliding-fee scale. You may apply for one of five discount levels, based on annual income and family size. The discount off of the standard charges will remain valid for one year after the date of application, unless you qualify for or secure insurance coverage in the interim. Work Hour Monday - Friday: 8:30 am - 5 pm Tuesday & Thursday evening hours: 5:30 pm - 8:30 pm Saturday Hours: 9 am - noon
